Bayou Vista Community Care is a four star rated CMS facility. Our facility provides twenty-four hour licensed nursing care as well as certified nursing assistants to assist our residents with their needs. We offer skilled and intermediate levels of care. Our goal is to help our residents achieve maximum levels of independence and optimum outcomes. Our plan of care is individualized to meet each resident's needs, so we can obtain the highest level of emotional, social, and physical wellbeing for our residents. At Bayou Vista, we offer Medicaid/Medicare private rooms to our residents. We offer our residents a well designed activity program, dietary services, and social services. Other available specialized services include our in-house physical therapy, occupational therapy, and speech therapy.
